Admission Requirements for Incoming Freshmen and Transferees
Starting a new academic journey is an exciting milestone, and ensuring that all admission requirements are in place is a crucial first step. To help students navigate this process with ease, the Institutional Student Programs and Services Office has outlined a clear and organized list of documents required for enrollment under the Higher Education Department (HED). This guide covers everything both incoming freshmen and transferees need to prepare.
For Incoming Freshmen
Aspiring first-year students are required to submit the following original documents:
- Form 138 or Report Card
- PSA-issued Birth Certificate
- Certificate of Good Moral Character
- One 2x2 ID photo with a blue background
- PSA-issued Marriage Certificate (only for married students)
Students transferring from another institution must provide:
- Honorable Dismissal or Certificate of Transfer Credential
- Official Transcript of Records (TOR) for evaluation
- PSA-issued Birth Certificate
- Certificate of Good Moral Character
- One 2x2 ID photo with a blue background
- PSA-issued Marriage Certificate (if applicable)
Enrollment Guide for Junior High School: Your Pathway to Grade 7 Success
As the academic year approaches, the journey into Junior High School begins with a simple yet vital step: preparing the right documents for enrollment. To help incoming Grade 7 students and transferees transition smoothly, the Admissions Office outlines the following comprehensive checklist for both general enrollment and Education Service Contracting (ESC) requirements.
General Admission Requirements. All incoming Grade 7 students and transferees are required to submit the following documents:
- Original Form 138/SF9 (Report Card)
- Original Certificate of Good Moral Character
- Photocopy of PSA Birth Certificate
- One 2x2 ID photo in collared shirt with a blue background
- F-137A/SF10 (Permanent School Record) from the previous school (to be requested by the School Admission Committee)
Important: Kindly organize all documents in one long brown envelope. Students are strongly advised to keep photocopies of every requirement, as submitted originals cannot be borrowed back once filed.
Education Service Contracting (ESC) Requirements. For students applying for ESC grants, the following documents must also be submitted:
- Original ESC Application Forms 1 & 2
- Photocopy of Form 138/SF9 (Report Card)
- Photocopy of PSA Birth Certificate
- Two 2x2 ID photos in collared shirt with a blue background
- Original and photocopy of ESC Certificate (only for transferees from other ESC-participating private high schools)
- Proof of Parent/Guardian Income
- If employed: Latest Income Tax Return (ITR) or Certificate of Tax Exemption
- If unemployed: Municipal Certification of Unemployment
Note: ESC-related documents must be submitted in a separate long brown envelope for processing.

Below is everything you need to know, broken down into simple steps so you can prepare with confidence.
General Admission Requirements
- All incoming Grade 11 students must prepare the following and submit them in one long brown envelope:
- Original Form 138 (Junior High School Report Card) This shows your academic achievements and is a key part of your admission.
- Original Certificate of Good Moral Character A reminder of your positive contributions and conduct in school.
- Photocopy of your PSA Birth Certificate Helps confirm your identity and age.
- Photocopy of your Junior High School Certificate of Completion A simple way to show you’re ready for the next academic chapter.
- One 2x2 ID picture Make sure you’re wearing a collared shirt with a blue background—say cheese!
- F-137A/SF10 (Permanent Record) Don’t worry—this will be requested by the School Admission Committee from your previous school.
Heads up! Once your original documents are submitted, they can't be borrowed—so be sure to keep a photocopy of each one just in case.
For SHS Voucher Applicants
If you’re applying for the Senior High School Voucher Program, here’s what to prepare (in a separate long brown envelope):
- Photocopy of Form 138 (JHS Report Card)
- Photocopy of your PSA Birth Certificate
- One 2x2 ID picture, same guidelines as above
- ESC Certificate (Original + Photocopy) if you were from an ESC-participating school
- QVR Certificate if you came from a private school that’s not part of ESC
