Pre-Admission Requirements and Procedures

Applicant for Collegiate Programs

  1. The applicant obtains an application form for the Intake Examination from the Guidance Services Office and pays the testing fee of ₱200.00.
  2. The applicant fills out and submits the forms along with the official receipt of the testing fee to the Guidance Staff.
  3. The applicant takes the exam on the scheduled date.
Reminders:
  • Testing room: 0-208 (adjacent to the Guidance Services Office)
  • Bring a valid ID with photo and signature.
  • Arrive at least 30 minutes before the test.
  • Exam duration: approximately 3 hours. Be physically, emotionally, and mentally prepared.
  • No one is allowed to leave during the exam, so attend to personal needs in advance. Bring snacks and water if needed.
  • NO WALK-IN EXAMINEES will be admitted.
  1. After the exam, the Guidance Staff will confirm the schedule for your Intake Interview.
  2. The applicant reports for the intake interview and brings the following:
    • One (1) Medical Certificate (from SAC Clinic)
    • Download your Clinical Laboratory Request Form
    • One (1) photocopy of PSA Birth Certificate
    • One (1) photocopy of High School Report Card or Transcript of Records (for Transferees)
    • One (1) photocopy of Certificate of Good Moral Character or Honorable Dismissal (for Transferees)
    • One (1) handwritten personal essay (on intermediate paper) addressing:
      • Your reasons for choosing SAC
      • Why you chose your course
      • Expected challenges in your course
      • Personal qualities, skills, or talents that can help you succeed
    • One (1) colored 2×2 ID photo
    • One (1) photocopy of marriage certificate (if applicable)
    • Duly accomplished Individual Inventory Record Form from the Guidance Office
  3. After the interview, proceed to enrollment and submit the following to the Registrar’s Office on enrollment day:
    • Student Registration Form signed by the Guidance Staff
    • One (1) original PSA Birth Certificate
    • One (1) original High School Report Card or Transcript of Records (for Transferees)
    • One (1) original Certificate of Good Moral Character or Honorable Dismissal (for Transferees)
    • One (1) photocopy of marriage certificate (if applicable)
