Admission Requirements

New students seeking admission to tertiary (college) level are required to submit the following:

  • Original copy of F-138-A / Report Card with the Final Grades, duly signed by the Principal and with notation “Eligible for Admission to College,” as fresh entrants to college;
  • Birth Certificate issued or authenticated by the National Statistics Office;
  • Certificate of Good Moral Conduct from the school last attended;
  • Certified True Copy of Marriage Contract for married students (for Catholics, church marriage certificate);
  • Certificate of Eligibility to Transfer with Certified True Copy of Grades, if transferee; and
  • Special Study Permit, if foreigner studying for less than a year or less than 18 years old. ACR – 1 Card if permanent resident, native born or with valid working visa, special investment resident visa (SIRV), special retirees visa (SRRV), 96 visa.

New students seeking admission to High School or Grade School level are required to submit the following:

  • Original Form 138;
  • Certificate of Good Moral Conduct from the school last attended;
  • Birth Certificate issued or authenticated by the National Statistics Office;
  • Baptismal Certificate (if Catholic); and
  • Parent’s Marriage Certificate.
